Clean Claim Rate
What is a clean claim rate?
A clean claim rate is the percentage of claims a practice submits that are accepted and paid on first submission, with no rejection, denial, or rework. Submit 1,000 claims in a month and have 880 paid without intervention and your clean claim rate is 88%. It measures whether your billing process works the first time, which is the single biggest lever on how fast you get paid.
Why first-pass success beats working the backlog
Every claim that fails on submission re-enters the cycle: someone has to identify it, correct it, resubmit it, and track it. That rework commonly costs $25 to $40 in staff time per claim and adds two to four weeks before payment. A practice at 75% is reworking one claim in four. A practice at 95% has freed its billing staff to work the genuinely difficult cases rather than clearing avoidable errors.
Finding where claims fail
Track rejections and denials by reason code and the pattern appears quickly: eligibility not verified at check-in, missing prior authorization, diagnosis and procedure codes that do not support each other, incomplete patient demographics, or stale payer requirements. Almost all of it originates at the front desk or in documentation rather than in billing. Claim scrubbing software catches format errors before submission, but it cannot supply an authorization nobody obtained.
