Explanation of Benefits

What is an explanation of benefits?

An explanation of benefits, usually called an EOB, is the statement a payer issues showing how it processed a claim: what was billed, what was allowed, what the payer paid, what was adjusted, and what the patient owes. The practice receives the equivalent document as a remittance advice. The EOB is the source record for posting payments correctly, and misreading it is how practice books drift out of line with reality.

Reading an EOB line by line

Each service line shows the billed charge, the allowed amount, the payer payment, the contractual adjustment, and the patient responsibility split into deductible, copay, and coinsurance. Reason codes explain any reduction or denial. The amounts must post to four different places in your books: cash received, contractual write-off, patient receivable, and any denied balance pending appeal. Collapsing them into one entry destroys the detail you need later.

Why posting discipline matters more than speed

When EOB amounts are posted as a lump sum, you lose the ability to tell a contractual adjustment from a denial or a patient balance from a write-off. That makes payer performance invisible and aging reports meaningless. Electronic remittance advice posts automatically if your system is configured with the right adjustment codes. Audit a sample of auto-posted remittances each month, because a miscoded adjustment type repeats silently across thousands of lines.
