Payer Mix

What is payer mix?

Payer mix is the breakdown of a practice's revenue by who pays for it: commercial insurance, Medicare, Medicaid, and patients paying directly. Two practices seeing the same number of patients can earn very different revenue depending on their mix, because every payer reimburses the same procedure at a different contracted rate. Payer mix is the single biggest driver of revenue per visit, and most owners have never seen theirs on paper.

Reading the mix for what it earns

Count the share of charges and the share of collections separately, because they rarely match. A payer that represents 30% of your charges may deliver 18% of your collections once contractual adjustments come off. Medicaid typically reimburses well below commercial rates, so a growing Medicaid share compresses revenue even as patient volume rises. Calculate net revenue per visit by payer and the comparison stops being abstract.

Shifting the mix deliberately

Owners have more control here than they think. Renegotiating a weak commercial contract, adding or dropping a plan, adjusting appointment availability, or expanding self-pay services all move the mix. Before dropping a payer, model the volume you would lose against the rate improvement on what remains. Track the mix monthly so a drift toward lower-reimbursing plans shows up in a trend line rather than in a bad year.
